Important Information for Individuals with Medicare Part B

Due to CMS mandates regarding services and billing, Dr. Choby is not currently able to accept patients with Medicare Part B or Medicare Supplemental Insurance even if they wish to pay out-of-pocket for services.

Patients over 65 with Medicare Part A plans may be seen if they do not have Part B or Medicare Supplement policies in place for coverage of outpatient services.
